Indonesian MOH Statement On 2 Recent H5N1 Cases

Four days ago, in Watching Indonesia: Media Reports Two H5N1 Cases In Tangerang, we saw unconfirmed media reports of two recent (fatal) H5N1 cases in Tangerang, very near the capital Jakarta.   Although these reports continued over the weekend, today the Indonesian MOH has finally posted a statement on their Sehat Negeriku (Healthy Mine Own Country) website.

 

Between 2006 and 2008, Indonesia was the undisputed leader in human H5N1 infections (122 cases & 112 deaths over 3 years), a dubious mantle claimed from Vietnam which led the pack between 2003 to 2005.  In 2009 Egypt took the title, and except for Cambodia’s brief run up in 2013, has held it since.

 

Over the past three years Indonesia’s bird flu cases have dwindled down to single digits, with only 2 cases reported in 2014. Their case fatality rate, however, remains among the highest in the world at 83.9% – more than double that we’ve seen in Egypt and a full 50% higher than the global average.

 

In today’s report we learn that a 40 year-old man and his son fell ill 4 days apart (son first) earlier this month, both were treated and released by different local hospitals – then readmitted as their conditions worsened – and both died within 48 hours of each other. 

 

Diagnosis of H5N1 did not come until each was at or very near the end of their life, a delay perhaps exacerbated by the decline in local H5N1 cases reported over the past 5 years. Media reports have linked their infections to a visit to the man’s sister’s farm where poultry had recently died, but their exact exposure remains under investigation.

Saudi Arabia: MOH Cancels Coronavirus Contracts Over Corruption Allegations

Saudi Arabia’s Ministry of Health has had a bit of a revolving door this past year, with long-time Minister of Health  Dr. Abdullah Al-Rabiah abruptly replaced by Dr. Adel bin Mohammed Faqih (see Saudi Minister Of Health Replaced) last April, presumably over his inability to get control over their growing MERS problem.

 

On December 8th, we saw the Saudi King Appoints New MOH Amid Sweeping Changes To Ministries, appointing Dr. Muhammad Bin Ali Al-Hayazie to replace Dr. Faqih after just 8 months.

 

About 24 hours ago the Arabic press – and twitter – began talking about a corruption probe at the Saudi Ministry of Health that reportedly involves more than a billion Riyals (270 million USD) of coronavirus-related expenditures.  Arabic machine translations being what they are (decidedly murky), I decided to wait until an English language report appeared to blog it.

 

While it only scratches the surface, and doesn’t name names or provide dates, the following report from Arab News at least provides us some idea of the scope of the investigation.

MoH contracts canceled over corruption

The Ministry of Health has suspended 38 contracts from a total of 148 for health-related programs on the suspicion of corruption, sources said.


Sources said the suspicions surrounded 38 contracts worth SR1 billion allocated to fighting the coronavirus. It was found the amount was not spent on preventive measures. The total amount of money spent on the contracts reached SR663 million.


A ministerial order has been issued to form an internal committee to verify these contracts and it has become apparent that some contracts had spent the allocated money in full, according to information revealed by sources.

It isn’t clear from this article exactly who did what, or under whose watch these `suspect’ contracts were let.   Assuming this isn’t handled in-camera, this may get very messy.  Except for the Arab News report above, I don’t mind much else in the English press about this.

 

If the official Saudi Press Agency (SPA) has covered this story, I’ve been unable to find it.

 

We are still waiting for the long promised case control study to be released by the Saudi Ministry, and with the height of MERS season soon to be upon us (April & May if last year is any guide), the sooner this gets resolved the better.

Egyptian MOH Statement On Today’s H5N1 Case

Earlier today, in Egyptian Media: 24th H5N1 Case Reported, I wrote about the latest H5N1 case reported out of Egypt.  The Egyptian Ministry of Health has posted the following statement which confirms that case, and the current count of 24 cases, and 10 deaths for 2014.

Egyptian MOH Announces 22nd H5N1 Case Of Year

After 3 weeks with nary a mention of bird flu, the Egyptian Ministry of Health has released their second update in three days, announcing their 22nd H5N1 case – and 10th fatality – of 2014. 

 

This brings to 18 the number of new cases reported since the middle of November – a substantial jump in H5N1 activity – as in all of 2013 only reported 4 cases and 3 deaths.


Whether these greater numbers indicate a change in the bird flu dynamic in Egypt, or simply represents improvements in surveillance and reporting over recent years is hard to know. 

 

For now, there are no indications of greater transmissibility of this virus, and all cases appear to have derived from direct contact with infected birds.

Egyptian MOH Update: 21st H5N1 Case Of 2014

It’s been nearly 3 weeks since we’ve seen an H5N1 update on the Egyptian Ministry of Health’s webpage (see here), and at that time the official count sat at 14 cases for the year.  Since then, we’ve a number of media reports of new cases, with Saturday’s report (see Media Reports Of New H5N1 Cases In Egypt) raising the count to 20 cases. 


Apparently this media count has been pretty accurate, as today the Egyptian MOH has posted a brief report on the 21st H5N1 case of the year, a 42 year-old man from Sohag who is reportedly in stable condition.

 

Of these 21 cases, 17 have been reported since the middle of November, with many of the recent cases coming out of Sohag Governorate.  While a definite uptick in cases (compared to the past couple of years), transmission still appears to be bird-to-human.

Saudi King Appoints New MOH Amid Sweeping Changes To Ministries

It was only last April that the long-standing  Minister of Health  Dr. Abdullah Al-Rabiah  was abruptly replaced by Dr. Adel bin Mohammed Faqih (see Saudi Minister Of Health Replaced), presumably over his inability to get control over their growing MERS problem.

 

Today we’ve word of a massive reshuffling of Cabinet Ministers in Saudi Arabia that includes replacements for the Ministers of Health and Ministers of Agriculture . . . among others.

 

I’ll try to get some information of the new Minister of Health Dr. Muhammad Bin Ali Al-Hayazie, meanwhile here is the the bulletin from the Saudi Gazzette, then the individual announcements from the Saudi Press Agency.

Saudi names new health minister to fight MERS

Hayazaa replaces Rabiah who was dismissed in April as concerns grew over kingdom's handling of MERS.

RIYADH - King Abdullah named a new health minister Monday to lead the fight against the MERS virus that has cost more than 350 lives in Saudi Arabia, after his predecessor was sacked.

Mohammad bin Ali bin Hayazaa al-Hayazaa replaces Abdullah al-Rabiah who was dismissed in April as concerns grew over the kingdom's handling of the Middle East Respiratory Syndrome coronavirus (MERS-CoV).

Before Hayazaa's appointment as part of a cabinet reshuffle, Labour Minister Adel Fakieh had been serving as acting health minister.

During the tenure of Fakieh, who promised "transparency" over MERS, the World Health Organisation said a surge in MERS cases had receded but urged continued vigilance.

Egyptian Govt Warnings On Bird Flu

Being a Friday, we may not see much official news out of Egypt until later today - but with 10 recent bird flu cases reported - the Arabic media is filled with reports of `suspected’ bird flu cases being isolated and tested (this is flu season, and most are probably not avian flu), and advice by the government to avoid contact with live birds.

 

A problematic warning, as it is estimated that 90% of rural Egyptian households keep poultry, and markets do a brisk trade in live birds in the cities.

 

In Egypt – live poultry - whether factory farmed or from backyard flocks, represents a major source of income, protein, and accrued wealth for millions of people. Take that away, and you risk destabilizing an entire region.

 

Nevertheless, since the H5N1 virus landed in Egypt in 2006, the government has announced a number of ambitious plans to move away from the raising or sale of `live birds’. Economic realities, political upheavals, and a strong public preference for live birds have, unfortunately, pretty much derailed these initiatives.


In 2007 headlines read Egypt To Close Live Poultry Markets, but of course . . .that didn’t happen.  In 2010 we saw Egypt’s Plan To Fight Bird Flu, that included a move away from increasingly ineffectual poultry vaccines (see Egypt: A Paltry Poultry Vaccine. and a ban on live bird sales. . . but that didn’t happen either.

The reliance upon poultry vaccines continues in Egypt, as it does in China and much of Asia, and as we’ve seen previously, this risks allowing `vaccine escape’ viruses to develop and circulate asymptomatically (see EID Journal: Subclinical HPAI In Vaccinated Poultry – China).

 

Today the media is carrying stories outlining the steps that the MOH is taking to prevent more cases, along with warnings that most of the recent H5N1 infections have come from `home raised’ poultry, suggestions that live bird trading should be halted, and advice that children should not be allowed to play with chickens.

Meanwhile Arabic media this morning continues to report on additional `suspect’ cases undergoing testing, and the raising of local governorate alert levels, such as Emergency Minya after the death of a fourth case of bird flu.

Mali MOH Statement On Newly Confirmed Ebola Case

While it doesn’t tell us anything more than we already knew, a statement has been posted on Mali’s MOH website (http://www.sante.gov.ml) that confirms the media reports of last night indicating a nurse had died from Ebola at a clinic in Bamoko after treating a patient from Guinea roughly 2 weeks ago.


Unlike Mali’s earlier case – that of a 2 year-old from Guinea who died in October – this case represents a local transmission of the virus.  There are media reports (unconfirmed) that a doctor at the clinic where this nurse worked who is also sick, and a large scale epidemiological investigation is underway.

 

Mali – a landlocked nation of more than 16 million people– shares a 500 mile-long border with Guinea and is highly dependent upon the ports of neighboring nations (like Guinea, Senegal & Cote D’Ivoire) to handle their import needs. 

 

In late October the government of Mali reiterated their decision to keep their borders open with Guinea, even after their first imported Ebola case was revealed, with President Ibrahim Boubacar Keita saying it would be impossible to  completely seal his country off from Ebola in neighboring Guinea.

 

With few exceptions, political borders around the world are more theoretical boundaries than practical ones, and are rarely an effective physical deterrent to the spread of infectious diseases. 

 

Which means that the longer that Ebola rages in Guinea, Sierra Leone, and Liberia the greater the chances are for it to seep into neighboring countries like Mali, Senegal, Cote D’Ivoire and Guinea-Bissau.

As part of the fight against Ebola virus disease in the country, health services conducted Monday, November 10, 2014 to investigate a health worker reported having had contact with a patient from Guinea made charge by a private clinic located in Commune IV in Bamako District.

 
During the investigation, a blood sample was taken Monday, November 10, 2014 on the subject. The results of the analysis of the samples were positive to Ebola Tuesday, November 11, 2014. The Mali and knows his second case of Ebola virus disease.


While her care were administered agent aged 25 health years sadly passed away Tuesday, November 11, 2014.

In these painful circumstances, the Government extends its condolences to the bereaved family and reminds the public that hygiene measures enacted remain effective means of disease prevention.


The Government assures that according to the protocol of care for patients Ebola virus in force, all measures are taken to identify the persons who had contact with the deceased. The premises of the private clinic who supported and the patient's home has been completely disinfected and placed under observation.


The Government informs the public that the health services, with the support of partners are hard at work to prevent the spread of the virus from this case linked to imported cases of Guinea.


It invites the population to remain calm, serenity but also and especially to the vigilance and compliance with hygiene measures enacted in the area.


Following the guidance of the President of the Republic, the Government of the Republic of Mali to thank all its partners for their assistance in his fight against this disease, reminds people to avoid unnecessary trips to the epidemic areas and observe hygiene measures and safety regulate the matter.

While we haven’ heard much about H5N1 in Egypt in recent months, we’ve continued to see FAO reports of outbreaks in Egyptian poultry, and so it isn’t terribly surprising that overnight the Egyptian MOH has issued the following (machine translated) statement on their 4th reported human H5N1 infection of the year.

Although H5N1 has taken a bit of a back seat to MERS-CoV, Ebola, and H7N9 it remains endemic in wild birds and poultry in both Asia and the Middle East, continues to evolve into new clades, and continues to pose a threat both to agricultural interests and (on occasion) to human health.


The most recent WHO Update: Influenza at the human-animal interface (H5N1), advises:

Overall public health risk assessment for avian influenza A(H5N1) viruses: Whenever influenza viruses are circulating in poultry, sporadic infections or small clusters of human cases are possible, especially in people exposed to infected poultry or contaminated environments. These influenza A(H5N1) viruses do not currently appear to transmit easily among people. As such, the risk of community-level spread of these viruses remains low.

 

That said, reserachers continue to watch H5N1, along with a growing cadre of recently emerged avian flu viruses (H7N9, H5N6, H10N8) for any signs that they are becoming better adapted to human physiology.

Saudi MOH: 1 New MERS Case

As usual, the Saudi MOH is providing minimal information, but after a summer with very few MERS cases reported, KSA has announced four widely scattered infections over the past 8 days. 

 

Previous cases have hailed from Taif, Riyadh, and Jubail while today’s case comes from Najram near the Yemen border.

 

With in excess of two million visitors expected to arrive for this year’s Hajj – which begins in less than 3 weeks -  any upsurge in MERS cases will be watched carefully.

KSA MOH Reiterates Camel Warnings On MERS

Although it has been a bit slow in coming, the acceptance that there is a strong connection between exposure to camels and contracting the MERS coronavirus seems to have finally taken hold in Saudi Arabia. Six months ago the Ministry of Agriculture was in full denial (see Saudi MOA Spokesman: Camel Link Unproven), and camel kissing briefly surged as an act of defiance.

 

For many Saudis, the idea that camels – a beloved national symbol that literally made settlement of that arid region possible – could carry a disease deadly to humans,  is simply unthinkable.   A concept made even harder to accept due to the widespread belief in the healthful effects of camel’s milk and urine in the treatment of disease.

 

In the face of mounting evidence, finally last May the Saudi Ministry Of Agriculture Issued Warnings On Camels, urging breeders and owners to limit their contact with camels, and to use PPEs (masks, gloves, protective clothing) when in close contact with their animals.

Since then we’ve seen a steady stream of new information implicating camels as an intermediate host for the virus, and a likely conduit to the human population.

Although the number of MERS cases has dwindled over the summer, yesterday two new cases were reported, and with the Hajj just three weeks away, the Ministry of Health apparently feels it is time for a reminder.  Today they have posted a notice on the Saudi MOH site, their Facebook page, and on twitter.

For transfer of coronavirus into human health Ministry warns against dealing with camels unprotected

Renewed Health Ministry warning to all citizens and residents to take necessary precautions in dealing with Apple. The Ministry showed that despite the decline in disease and thankfully, the expected individual cases of disease caused by direct exposure to secretions of camels without wearing a muzzle and gloves and cover the body. The Ministry said that these individual cases may be a source of transmission of the virus to others health workers at hospitals or close contacts at home.

The Ministry showed that research conducted by scholars of the Saudis last November proved without doubt the role of camels to transport human coronavirus to which scientific studies have shown the presence of the virus in the mucus and saliva of camels particularly affected by symptoms such as runny nose and fever. The Medical Council has issued an advisory of the Ministry of health warnings and stressed the need for taking precautions within the camel barns during direct dealing with, as well as within the slaughterhouses. These precautions include wearing protective gloves and gag curtain of flesh. And calls upon the Ministry of health of the citizens and residents of the need to follow those precautions and not lax in maintaining their health and the health of those around them.

An Unexpected Announcement From The Saudi MOH

Dr. Ziad Memish – Deputy Minister of Health for Saudi Arabia – has been the very public face of the Saudi’s efforts (for better or worse) to quantify and control the MERS Coronavirus.  His name has been attached to many of the studies that have been conducted by labs around the world on this emerging coronavirus (see here, here, and here).

Despite this notoriety, he has also had to fend off growing international criticism on the slow progress being made against the spread of MERS, and very recently publicly stated he was `surprised’ at the criticisms being lodged.



Today, in the briefest of statements on the Saudi MOH website, we get word that suggests that Dr. Memish is no longer acting as the Deputy Minister of Health.

Iranian MOH Statement On Testing Of `Suspected’ MERS Cases

The story has been filtering out via the media for a couple of hours (see Crofsblog Iran: Six other suspected MERS cases), but exactly what the status of these additional cases really is, isn’t exactly clear. Previously we’d been told that two cases had tested positive, and four others were suspected and undergoing testing. 

 

This evening the Iranian MOH has issued a statement in Farsi- which doesn’t translate particularly well – that seems to indicate as many as 9 more cases are being tested. 

 

The headline denies these 9 cases are MERS-CoV positive, but the body of this syntax challenged machine translation suggests test results are still pending.  

 

Hopefully we’ll get a better read on the situation over the next day or so.

KSA MOH: 16 New MERS Cases

After several days of single-digit reports, today’s MOH announcement contains 16 new cases, and 5 deaths.  Cases hail from Riyadh (9), Jeddah (5), and Medina (2) and range in age from 2 to 72.


For the first time since Saturday, this report also counts asymptomatic cases (3), which have been noticeably absent during the past few days.

 

Although several of today’s cases are listed as being of `working age’ and `contacts of a confirmed case’, none are expressly identified as Healthcare workers.  We’ve not seen any health care workers specifically mentioned in these reports for well over a week, although some appear likely to be HCWs.

  5. A 36 year old woman developed respiratory symptoms on May 8, 2014. She was admitted to a government hospital on May 11, 2014. Her condition is stable.

C. AlMadinah:

  1. A 2 year old child with congenital anomalies developed respiratory symptoms on Apr 19, 2014. She was admitted to a government hospital on Apr 25, 2014. She is in the ICU. May Allah grant her a speedy recovery.
  2. A 41 year old woman who has been in contact with a confirmed case. She does not have any symptoms.

Deaths from previously reported cases:

  1. A 55-year-old man passed away on May 13, 2014.  He was previously recorded as a confirmed case in Jeddah. May Allah have mercy upon him.
  2. A 57-year-old man passed away on May 13, 2014.  He was previously recorded as a confirmed case in Jeddah. May Allah have mercy upon him.
  3. A 60-year-old woman passed away on May 13, 2014.  He was previously recorded as a confirmed case in Riyadh. May Allah have mercy upon her.

Recovered and discharged cases that previously tested positive and are now negative:

  1. A 42-year-old woman was discharged from a government hospital in Jeddah.
  2. A 56-year-old man, who was in home isolation in Madina, now tested positive for MERS Corona.
  3. A 58 year old woman was discharged from a government hospital
