Fujian Province Health: 15 New H7N9 Cases

Fujian province ended the second wave of H7N9 with just 22 reported cases, and until the January 7th announcement (see Fujian Province Reports 6 H7N9 Cases In December) had remained quiet throughout the fall. We saw Hong Kong’s CHP update their count for Fujian to 28 a week later. 

 

Unfortunately, the absence of onset dates, ages and genders in these reports makes keeping a decent line-listing, and good track of the totals, almost impossible.

 

So today, when we get a report from the Fujian Provincial Health Department like the one below, we can’t double check these cases, and we are forced to the update at face value. Since they already reported 6 cases in December, presumably all of these cases occurred since the first of the year.

 

This year the province were found in 15 cases of human infection with H7N9 cases

As of January 15 24, were reported across the province this year, 15 cases of human infection with the H7N9 cases. Among them, five cases of Fuzhou, Xiamen, six cases, two cases of Putian, Quanzhou, Zhangzhou and one case each. 2 deaths (Xiamen, Putian, one case each).

Experts advise, H7N9 flu is preventable, controllable and treatable, is a time when flu season, try not to direct contact with the public and self-slaughter handling live poultry, birds do not buy products of unknown origin, do not eat dead birds; to ground Wash your hands, wash your hands with soap after contact with birds; poultry and eggs should separate raw and cooked, cooked fully cooked before eating; if fever, cough, sore throat, general malaise and other symptoms, should wear a mask to the hospital.         While not exactly a wealth of information, the China News Network has the following report which seems to confirm that these are, indeed, all new cases.

        BEIJING, Jan. 16 Fuzhou Power (Lin Chun Yin) Health and Family Planning Commission of Fujian Province on the 16th evening briefing, as of 15 January 24, this year, the province reported a total of 15 cases of human infection with the H7N9 cases, including two cases of dead.

        Informed that found no clusters of cases, 15 cases of human infection with H7N9 cases, including five cases of Fuzhou, Xiamen, six cases, two cases of Putian, Quanzhou, Zhangzhou each one case, where Xiamen, Putian, one each deaths.

        Fujian provincial government recently held a special meeting to be deployed to do H7N9 prevention and control work, health and family planning at all levels, agriculture, market supervision and management departments have started joint prevention and control mechanism, the full implementation of epidemic prevention and control measures focus on strengthening the live bird market supervision.

        Fujian Health Emergency Planning Office officials said, grasp the live bird market regulation is currently focused on the prevention and control, health conditions unqualified live animal markets are closed, live bird markets and slaughter point to strictly implement the "day one cleaning a week a disinfectant, January 1 Rest "and other measures, to be completely emptied before the rest, thoroughly cleaned, disinfected, to prevent cross-

         

        Assuming these reports are accurate Fujian province has reported nearly as many cases (n=21) in the past 6 weeks as they did during the first two epidemic waves.

        Fujian Province Reports 6 H7N9 Cases In December

        One of the more encouraging aspects of 2013’s H7N9 epidemic in China was the unprecedented openness on the part of the Chinese government in announcing cases, releasing studies, and keeping the outside world well informed on this newly emerged avian threat.   

         

        It was a dramatic, and praiseworthy change from the tactics employed during the 2003 SARS outbreak, and it was followed in much the same fashion the following winter for the second wave.


        But this year, not so much.

         

        We’ve seen a smattering of very brief case reports issued from a few provinces, but rarely with the kind of detail we became used to during the first two waves. Onset dates, routes of likely exposure, and follow up reports have been few and far between.


        And quite frankly, there’s been a growing feeling among the newshounds following these reports that we’re not seeing all of the cases reported in a timely fashion.

         

        Supporting this concern, this morning there was a report in the Chinese press that last month – December of 2014 – Fujian Province recorded 6 H7N9 cases. The only one previously announced was on December 3rd (see Fujian Province: 1 H7N9 Case Reported), meaning that we have no details at all on at least 5 cases.

        Reporters learned from the Fujian Provincial Health Planning Commission last month, the province reported a total of six cases of human infection with the H7N9 bird flu cases and no deaths reported. Health experts said the Planning Commission of Fujian Province, people infected with H7N9 is an acute respiratory infection caused by the H7N9 virus. At present, evidence suggests that exposure to live poultry markets are risk factors for the incidence of human infection with H7N9, carrying the virus in poultry and their droppings, secretions of human infection with H7N9 virus may be the source of infection.

         

        Admittedly, not much of a report.  But Sharon Sanders on FluTrackers has found another confirmation, via a monthly summary of communicable diseases published by the Fujian Provincial Health Department.

        December 2014 (at 0:00 on December 1, 2014 to December 31 24), the province reported a total of A and B infectious diseases 11,363 cases and 19 deaths. This month except plague, cholera, SARS, polio, highly pathogenic avian influenza, rabies, Japanese encephalitis, anthrax, epidemic cerebrospinal meningitis, whooping cough, diphtheria, neonatal tetanus, leptospirosis, schistosomiasis no case reports, the rest are being reported. The top five reported incidence of A and B infectious diseases: viral hepatitis, syphilis, tuberculosis, gonorrhea, HIV, accounting for 98.29% of the total incidence; infectious disease deaths reported as: AIDS, tuberculosis. December, the province reported a total of six cases of human infection with the H7N9 cases, no deaths.

        December 2014, the province reported a total of 11,036 cases of class C infectious diseases, no deaths. Reported incidence of the top five diseases were: other infectious diarrheal diseases other than cholera, bacillary and amoebic dysentery, typhoid and paratyphoid, foot and mouth disease, influenza, mumps, acute hemorrhagic conjunctivitis, accounting for 99.88% of the total number of class C infectious diseases.

         
        While there is no mention on the front page of the Fujian Provincial Health & Family Planning’s website of these cases, yesterday they did feature prominently the following advisory on avoiding H7N9.

         

        Do routine prevention, away from people infected with H7N9

            People infected with H7N9 is an acute respiratory infection caused by the H7N9 virus. At present, evidence suggests that exposure to live poultry markets are risk factors for the incidence of human infection with H7N9, carrying the virus in poultry and their droppings, secretions of human infection with H7N9 virus may be the source of infection. Everyday life, experts say, away from people infected with H7N9, take the following precautions are critical:

           (1) daily life should avoid direct contact with live poultry, birds or their droppings, especially disease (dead) birds; if it has contact Wash hands with soap and water as soon as possible. Children should avoid direct contact with poultry and wild birds. If you find disease (dead) birds, animals, do not deal with on their own, should report the authorities.

           (2) Do not buy live poultry slaughtered on their own, do not touch, do not eat sick (dead) poultry, meat, do not buy without quarantine certificates fresh, live, frozen poultry and their products.

           (3) raw poultry, meat and eggs must be cooked fully cooked.

           (4) attention to food hygiene, food processing, food process, there must be raw and cooked separately to avoid cross contamination, handling raw poultry, meat chopping board, knives and containers can not be used for cooked food; in processing raw poultry After the meat and eggs thoroughly wash their hands.

           (5) a healthy lifestyle is very important for the prevention of this disease. Usually should strengthen physical exercise, plenty of rest, avoid over exertion; no smoking, wash hands, pay attention to personal hygiene, cover your nose and mouth when sneezing or coughing.

           (6) children, the elderly, the infirm and those with chronic diseases should be minimal to crowded public places.

           To wear a mask (7) when the doctor or go to the hospital to visit patients.

           (8) If fever and respiratory symptoms should wear a mask, as soon as possible to medical institutions, and remember to tell your doctor before the onset of whether the travel or contact history with poultry.           While we are always cognizant that surveillance and reporting can only pick up a portion of the actual number of cases, over the past couple of years we’ve come to rely heavily upon the excellent reporting by China’s provinces in providing us a pretty good real-time barometer of their H7N9 epidemic status.


          It would be a great loss if this year should prove any different.

          Fujian Province: 1 H7N9 Case Reported

          Although we’ve only seen a handful of H7N9 cases reported this fall, the past couple of weeks have produced reports from Zhejiang, Guangdong, and the Xinjiang Uygur Autonomous Region.  Today, from the Fujian Provincial Department of Health we get the following brief report:

           

          H7N9 confirmed cases in the province reported one case of human infection

          Yemou patient, male, 27 years old, currently living in Fuqing City.  December 3 confirmed human infection of H7N9 confirmed cases, patients present with severe illness, isolation and treatment hospitals in Fuzhou.

           

          Previously, Fujian province had reported 22 cases of H7N9 over the first two waves.   This is their first report of the 2014-15 winter season.


          Last year, we saw a smattering of H7N9 cases in November and December, and a huge increase during January and into February.

          Zhejiang, Fujian, Guangdong & Hong Kong Announce New H7N9 Cases

          You know its a busy morning when the daily tally of H7N9 is the sixth blog of the day.


          Anyway, with a great deal of help from the newshounds at FluTrackers and Sharon Sander’s constantly updated and invaluable H7N9 Line Listing, we’ve five new cases reported out of Mainland China, along with the earlier reported Hong Kong Fatality (see CHP Announces 4th H7N9 Case In Hong Kong).

           

          First stop, Zhejiang Province which reports three new cases today:

          Zhejiang Provincial Health and Family Planning Commission January 29 briefing, the province today added three cases of human infection with H7N9 avian influenza.

          1, patients Moumou, male, farmer, aged 37, Wencheng County of Wenzhou. January 28 confirmed human cases of avian influenza H7N9 infection. Is now in critical condition at a hospital in Wenzhou treatment.


          2, patients Zhu Moumou, male, individuals, aged 60, Xiaoshan District, Hangzhou people. January 28 confirmed human cases of avian influenza H7N9 infection. Now the condition is severe, treatment in a hospital in Hangzhou.


          3, the patient Gu Moumou, male, retired, 63 years old, Hangzhou City people. January 28 confirmed human cases of avian influenza H7N9 infection. Now the condition is severe, treatment in a hospital in Hangzhou.

           

          Next stop, Guangdong Province, which yesterday reported three relatively young cases (ages 43, 41 & 31), reports a rare teenage infection today.

          Patients Zhang, male, 17 years old, high school student, who now lives in Tianhe District, Huangpu Road Street Village Street. January 29 confirmed cases of human infection of H7N9 avian influenza, the patient's condition is currently stable in Guangzhou City, the designated hospital admission.

           

          Last stop is Fujian province, where just as unusually, a toddler is reported infected by the virus.

          Patients Moumou, male, 2 Suiyu, now living Xiang'an District. Currently patients with mild disease, in recovery, isolation and treatment at a hospital in Xiamen. The case for the province routine influenza surveillance sentinel hospitals found cases. (Emergency Management Office)

           

          Although reported H7N9 cases have skewed heavily towards older patients - among those younger cases identified - we’ve seen relatively mild illness, which may be affecting  the detection rate.  Simply put, those with mild illnesses are less likely to seek medical care, get tested, and end up counted. 

           

          Whether younger people are actually infected less often than older people is something that probably can’t be firmly established until good, community wide seroprevalence studies can be conducted.

           

          We may see additional cases announced later in the day, in which case I’ll post an update.

          Fujian Province Announces Additional H7N9 Case

          In addition to the 10 cases reported earlier this morning (see Beijing, Zhejiang, Guangdong All Announce New H7N9 Cases) we’ve the following statement from the Fujian Ministry of Health website.

          Zhejiang, Guangdong, Fujian Provinces Report New H7N9 Cases

          While we may hear of additional cases later in the day, as of 7 am EST three Chinese provinces (Zhejiang, Guangdong, Fujian) have reported a total of 7 new H7N9 cases today, bringing to 48 the number of new cases reported since the first of the year.

           

          Our first stop, Zhejiang province, which for the 10th day in a row is reporting new cases, bringing that province’s total since the first of the year to 21 cases.

          Zhejiang Provincial Health and Family Planning Commission January 18 briefing, the province today added three cases of human infection with H7N9 avian influenza.

          1, the patient Xiao Moumou, male, 35 years old, farmer, Cixi, Ningbo people. January 16 confirmed human cases of avian influenza H7N9 infection. Is now in critical condition, a hospital for treatment in Ningbo.

          2 patients Moumou, female, 72 years old, retired, Binjiang District, Hangzhou people. January 17 confirmed human cases of avian influenza H7N9 infection. Is now in critical condition, in Hangzhou, a hospital for treatment.


          3, the patient Zhong Moumou, male, 69 years old, farmer, Shaoxing Paojiang people. The 17th confirmed human infection of H7N9 avian influenza. Is now in critical condition, a hospital for treatment in Shaoxing.

           

          Our second stop is Guangdong Province, which sits next to Hong Kong.  They too are reporting 3 new cases, bringing their total since the first of the year to 17 cases.

          Health and Family Planning Commission of Guangdong Province on January 18 briefing, the province confirmed three cases of human infection with the H7N9 avian influenza cases were reported two cases in Guangzhou, Shenzhen, reported one case.

          Case 1 Panmou, female, 5 years old, currently residing in Baiyun District of Guangzhou City. January 14 onset, January 18 confirmed cases of human infection of H7N9 avian influenza, patients with mild symptoms currently in Guangzhou designated hospital admission.

          Case 2 Yang, female, 83 years old, retired people, now living in Liwan District of Guangzhou City. January 2 disease, January 18 confirmed cases of human infection of H7N9 avian influenza, the patient is currently in critical condition in the Guangzhou designated hospital admission.

          Case 3 Zhou, male, 62 years old, retired people, now living in Futian District, Shenzhen. January 6th disease, the patient's condition is currently stable in Shenzhen designated hospital admission.

           

          Of note, the first case listed is a 5 y.o. with mild symptoms, which is the first case under the age of 20 we’ve seen reported in 2014.  By my count, only a little over 3% of the cases (n=7) reported to date have been under the age of 20. For the most part, these younger cases seem to experience less severe symptoms than do older patients. 

          While it certainly appears that younger children are less likely to be infected, it may also be a factor that younger patients seem to experience much milder symptoms, and are therefore are less likely to seek medical aid and get tested.

           

          Our last stop is Fujian Province, which is reporting a single case today. The Health and Family Planning Commission of Fujian Province website appears to be offline right now, but we have the following report from Xinhua News.

           

          New human H7N9 case in E China

          FUZHOU, Jan. 18 (Xinhua) -- A new human case of H7N9 bird flu was reported in east China's Fujian Province on Saturday, bringing the total number of cases in the province to seven.

          The patient is a 55-year-old man surnamed Lu, a motorcycle driver from Yongning Township in the city of Shishi, according to a statement from the provincial health and family planning commission.

          Lu is being treated in a local hospital and is in a critical condition.

          Human H7N9 cases have also been reported this year in Jiangsu, Zhejiang and Guangdong provinces and Shanghai municipality.

           

          While this daily parade of fresh cases is obviously a concern, thus far we haven’t seen any epidemiological evidence to indicate the virus is transmitting between humans in an efficient or sustained manner.

           

          That said, there are a great many unanswered questions regarding how this virus is infecting humans, the incidence of mild or asymptomatic cases in the population, and its reservoir host.

          Fujian, Guangdong, Zhejiang Provinces Report New H7N9 Cases

          Our morning roundup of newly reported H7N9 cases numbers six  from three provinces.  Although it is now Thursday evening in China, it is always possible we’ll see more before the day is out.

           

          Our first stop, is Fujian Province, where two cases have been reported in the past 24 hours, making four cases so far in 2014.

          Health and Family Planning Commission of Fujian Province January 15 briefing, the province 15 Nisshin confirmed one case of human infection with the H7N9 avian flu, the patient had a certain, male, 30 years old, currently residing in Quanzhou Nanan, engaged in stone processing, is currently in critical condition compared, In Quanzhou, a hospital for treatment. Patients Xu Moumou, male, 60 years old, self-employed businessmen, now living in Quanzhou Luojiang present serious condition, a hospital in Quanzhou, isolation and treatment.           Our next stop is Guangdong Province, adjacent to Hong Kong, where two new cases (1 fatal) have been reported as well. A second death, that of an earlier announced case is reported as well.

          Case 1 Fumou, male, 59 years old, retired, currently residing in Haizhu District of Guangzhou. January 16 confirmed cases of human infection of H7N9 avian influenza, is currently in a stable condition in Guangzhou City, the designated hospital admission.

          Case 2 0000 A, F, 76 years old, retired people, now living Foshan. January 16 confirmed cases of human infection of H7N9 avian influenza, is currently in critical condition, in Foshan City, the designated hospital admission.


          Also, one case of human infection with the H7N9 avian flu Yangjiang January 6 reported died due to respiratory and circulatory failure, on at 23:10 on January 15 death.

           

          And our last stop is Zhejiang Province, which for the 8th day running, has also reported new cases.

          1, the patient Tan Moumou, female, 20 years old, farmer, now living in Hangzhou Xiaoshan District. January 15 confirmed human cases of avian influenza H7N9 infection. Now is critical, in Hangzhou, a hospital for treatment.

          2, patients Qiu Moumou, male, 58 years old, farmers, Zhejiang Jiaojiang people. January 16 confirmed human cases of avian influenza H7N9 infection. Now is critically ill in a hospital in Taizhou treatment.

          While we continue to see sporadic (and for the most part, widely scattered) cases of H7N9 popping up in Eastern China, thus far we’ve seen no epidemiological evidence of sustained or efficient human-to-human transmission.  

           

          There are, however, many unanswered questions regarding how this virus is transmitting to humans, and the incidence of mild or asymptomatic cases is largely unknown.

           

          A few small family clusters have been reported, and laboratory testing suggests this virus is better adapted to mammals than other avian viruses we’ve seen (see mBio: H7N9 Naturally Adapted For Efficient Growth in Human Lung Tissue), so the concern remains that given enough time and opportunities, H7N9 could better adapt to human physiology.

          Fujian Province Reports H7N9 Fatality

          My thanks to Arkanoid Legent for tweeting the Xinhua report which led me to the Fujian province announcement (see below) of the death of 37 year-old male with multiple comorbidities – admitted to the hospital on Wednesday – due to the H7N9 virus. 

           

          This marks the first reported case in Fujian Province in 2014, although last year they reported five cases.

           

          First the Xinhua report, followed by the Fujian Health Ministry announcement.

          FUZHOU, Jan. 10 (Xinhua) -- A 38-year-old man died from H7N9 bird flu on Friday in east China's Fujian Province, the provincial health and family planning commission confirmed in a statement.

          The man, surnamed Zeng, was from Hui'an County of Quanzhou City. He was confirmed to be infected with the virus in the early morning of Friday and died in hospital in the morning, according to the statement.

          He was admitted to hospital on Wednesday.

          This was the first human case of H7N9 in Fujian this year.

          Human cases of the bird flu virus have also been reported in the provinces of Zhejiang, Jiangsu and Guangdong so far this year.

          Patients had a certain, male, 38 years old, lives in Quanzhou Hui'an. January 3 coughing, wheezing and other symptoms, not treatment, a history of tuberculosis, pneumoconiosis and other medical history. January 8 due to exacerbations stay in a hospital for treatment of Quanzhou. January 10 morning, Quanzhou CDC laboratory testing of human infection with H7N9 avian influenza virus nucleic acid positive. Review by the provincial CDC, confirmed cases of human H7N9 avian influenza infection diagnosed. The patient died on the 10th through the morning of the death.

          Ministry of Health and Family Planning Commission reminder, residents pay attention to personal hygiene, washing hands frequently, do not spit; strengthening exercise, pay attention to nutrition, to ensure adequate sleep, strengthen body resistance; symptoms of acute respiratory infection, fever, cough, namely to the hospital treatment.           This, along with two earlier reports (Guangdong Province Announces Two `Mild’ Cases Of H7N9 & Zhejiang Province Announces Two More H7N9 Cases) brings today’s total number of announced cases out of Mainland China to five.

          H7N9: Fujian Province Reports First Case

          Despite the announcement two days ago by the Chinese government that case information would be released on a weekly basis, I’m pleased to report we continue to see case reports from individual provinces. 

           

          Yesterday Jiangxi province was added to the geographic range of this virus, and today, we learn the virus has spread even further south with this  confirmed case reported in Fujian Province.
